Minor Procedures Referral E-Form

Dear Colleagues,

It is with pleasure that we are writing to inform you of the launch of our minor procedure clinic. Please find attached our referral form, if there are any procedures that you cannot find on our form, please indicate it in the “other” section.

Things that should not be referred:

• Lesions on the neck, nose, eyes, or ears

• Patients who are very particular about cosmetic outcomes

• Large lesions that would require skin grafts or extensive suturing.

• Referrals for diagnostic clarification – we have no extra training in dermatology, the clinic will strictly do an assessment and followed by the procedure

Of note, the consults would be billed as an out-of-basket A905 consultation and would not result in any financial penalties.
